Every chronic disease management programme at Fortify Fitness follows a rigorous, four-stage clinical process. This is not a generic exercise programme with a health label — it is an individualised clinical service designed around your specific diagnosis, history, and goals.
Your journey begins with a comprehensive intake assessment. We review your medical history, current medications, recent pathology or imaging results, and any GP or specialist referral notes. We use validated assessment tools to establish your baseline — including AxIT strength and power testing and Tanita body composition scanning where appropriate — so your programme is built on real data, not assumptions.
Using your assessment findings, we design a programme that accounts for the specific physiological demands and contraindications of your condition. For cardiovascular clients this means heart-rate-guided aerobic progression. For musculoskeletal conditions, graded loading within your pain-free range. For metabolic conditions, combined aerobic and resistance modalities calibrated to glycaemic control. Every detail is condition-specific.
All sessions are supervised by an ESSA-registered Accredited Exercise Scientist. We monitor your response to exercise — including heart rate, perceived exertion, pain levels, and objective output measures — and adjust the programme as your capacity changes. This is not personal training with a chronic disease label; it is clinical exercise delivery by credentialled professionals.
